Position Overview

We are seeking a RN / Registered Nurse who enjoys travel, a flexible work schedule, working independently, and providing patient care in the home setting. You will have the satisfaction of knowing you’re ensuring the patient receives optimal care by assessing, planning, implementing reevaluating and revising care and coordinating resources. Among other responsibilities, you will update and maintain patient records and implement orders for medications and treatments.

As a Registered Nurse RN, your job duties will include:

• Administer nursing care to patients in a professional and caring manner
• Provide advice and emotional support to patients and their family members
• Evaluate and establish patient treatment plans
• Monitor and report changes in patient symptoms or behavior
• Communicate with collaborating physicians or specialists regarding patient care
• Educate patients about health maintenance and disease prevention
• Maintain accurate patient medical records
• Attend and participate in multi-disciplinary patient case conferences, staff meetings and in-service education programs as assigned
• Cooperate with on call schedule, weekends and after-hours visits, as necessary

. Requirements:

• RN license in Tennessee in good standing
• Previous home health, private duty skilled nursing, hospice, hospital or case management experience preferred
• Familiarity with electronic medical records / EMR – MatrixCare and OASIS a plus
• Ability to build rapport with patients
• Strong problem solving and critical thinking skills
• Ability to thrive in a fast-paced environment
• Reliable transportation, valid driver’s license and current auto insurance
