As a Clinical Field Operations Manager II, responsibilities include, but are not limited to:
Own responsibility for attaining clinical performance goals including hospitalization reduction, goals of care discussion, and home visit goals.
Provide day-to-day oversight and management of all clinical staff in the market.
Lead professional development and education efforts for a growing team of palliative care clinicians (NP, PA, SW, RN) including supervising training, performing ride-along visits with staff, mentoring/coaching staff, and implementing performance improvement plans for team members as needed.
Communicate performance feedback to palliative care clinicians regularly, including through twice annual performance reviews.
Lead a weekly interdisciplinary team (IDT) meeting focused on creating a positive team culture, discussing high-risk patients, providing clinical education and discussing operational needs.
Establish and maintain positive, caring relationships with executives, managers, physicians, non-physician providers, support staff, and patients/families
Assist with clinician recruitment, onboarding, and training.
Assist the Vice President of Operations with education and outreach efforts to primary care physicians and specialists.
Assist the Vice President of Operations with health plan communication and coordination.
Assist the corporate clinical leadership team with development and implementation of new policies and processes.
Make as-needed urgent visits to acutely ill patients (please refer to general direct patient care responsibilities).
Qualifications
Requirements
Must have or be eligible for appropriate APP licensure.
DEA possession or eligibility
Must have reliable automobile, valid driver’s license and minimum state required liability auto insurance; APPs travel in personal vehicle with reimbursement for mileage
Graduate of an approved Nurse Practitioner program or Graduate of a Physician Assistant program accredited by the Accreditation Review Commission on Education for the Physician Assistant (ARC-PA)
Preferred Qualifications:
A successful candidate will have previous supervisory or management experience
Previous experience in home care, hospice, palliative care, or geriatrics strongly preferred; other complex patient health care experience also beneficial.
