Responsibilities The Licensed Practical Nurse (LPN) Ambulatory Care and LPN, Advanced, provides direct and indirect patient care under the direction of the Registered Professional Nurse, Nurse Practitioner, Physician’s Assistant, or Physician. The LPN functions as part of the interdisciplinary health care team. The LPN, Advanced serves as a resource for other team members and is active in the orientation of new staff. Collects and records relevant patient health data accurately, with no more than 3-5 exceptions, as measured by random supervisory chart review and/or direct supervisory observation and co-worker feedback. Evaluates health data to identify patient problems, with no more than 2-3 exceptions, as measured by random supervisory chart review and/or direct supervisory observation. Implements and documents specified interventions with no more than 3-5 exceptions, as measured by random supervisory chart review and/or direct supervisory observation. Identifies practice problems and consistently seeks solutions through appropriate communication channels. Delivers care in a non-judgmental and non-discriminatory manner that is sensitive to patient diversity as measured by no significant, substantiated patient or peer complaints received by the manager/supervisor. Demonstrates operational problem-solving ability through written exemplar, patient/family feedback, co-worker report or direct managerial observation. Qualifications Education: Graduation from an accredited Practical Nursing program is required Experience: Minimum of 2 years experience in current area of ambulatory practice Minimum of one year with Bassett Healthcare Licensure/Certificates: Current NYS Licensure as a Practical Nurse Additional credentials are determined by specific work unit Skills: Ability to determine patient status and take appropriate action. Proficiency is required nursing skills relevant to area of practice. Ability to multi-task effectively in a rapidly changing environment. Ability to lead approved work unit initiatives. Services a resource to all staff and mentor to LPN Ambulatory Care. Ability to lead approved work unit initiatives. Physical Requirements: Walking, standing, squatting and kneeling required throughout the shift. Ability to lift and position patients required. Category I risk exposure as defined by OSHA Must be able identify and differentiate colors.
