Since 1985, BHCHP’s mission has been to ensure unconditionally equitable and dignified access to the highest quality health care for all individuals and families experiencing homelessness in greater Boston. Over 12,000 homeless individuals are cared for by Boston Health Care for the Homeless Program each year. We are committed to ensuring that every one of these individuals has access to comprehensive health care, from preventative dental care to cancer treatment. Our clinicians, case managers, and behavioral health professionals work in more than 60 locations to serve some of our community’s most vulnerable—and most resilient—citizens.
From our earliest days as a program, we have always sought to do work that is transformational: recognizing our shared humanity; centering dignity, compassion, mutual respect and supporting the right of every individual to access the highest levels of health care and every staff member to reach their fullest potential. We continue to be committed to building bridges and breaking down barriers, including systemic racism which harms us all. Job Summary:
NOW OFFERING A $4,000 SIGN ON BONUS
We are seeking an experienced Registered Nurse to work in collaboration with other members of the Family Team to ensure access to high quality healthcare for families and youth experiencing homelessness and housing instability in the Greater Boston Area. As the PD RN, you would be responsible for addressing all health-related issues for clients in the target population who seek assistance from the Family Team. This is work is accomplished by assessments, interventions, referrals, and consultations. Family Team services are provided by a multidisciplinary staff in a variety of shelters, scattered site housing, soup kitchens, day care centers, domestic violence shelters, transitional programs, adult shelters and through street outreach.
The Family Team is part of an integrated health care program for the people experiencing homelessness. The services provided include primary and episodic care for adults and children, mental health and substance use treatment, reproductive health, nutrition, referral to specialty care, social service advocacy and support, and case management.
Responsibilities:
Provide comprehensive assessments of all patients at assigned sites who seek services. Ensure that all sites and residents understand the services available to them through the program.
Work as a member of a multidisciplinary team planning care to address all health-related issues for families and youth experiencing homelessness.
Develop and implement interventions in collaboration with the clients and appropriate staff members.
Monitor the patients’ ability to follow care/case management plan. Provide assistance in addressing barriers to accessing care including, but not limited to access to transportation, childcare, language interpretation.
Evaluate effectiveness of interventions in collaborations with the patients and appropriate staff members.
Provide health education to individual patients and site staff based on collaborative assessment of need. Focus on health promotion and disease prevention. Conduct screenings and vaccination clinics as needed/requested.
Qualifications:
Registered Nurse; current Massachusetts license required; At least two years of generalist/medical-surgical/community nursing.
Knowledge of common health problems experienced by individuals and families experiencing homelessness desired, including mental illness and substance use disorders; Demonstrated interest in working with an undeserved population.
Bilingual English/Spanish or English/Haitian Creole preferred.
Pediatric nursing experience required; outreach experience preferred.
Valid driver’s license and car required.
Compensation and Benefits:

The compensation starts at $37.82 per hour and increases based on years of experience.
