We’re seeking a compassionate and experienced community support coach to join our team and help empower individuals within our community to reach their full potential. As a community support coach, you’ll work closely with individuals to assess their needs, develop personalized support plans, and connect them with community resources to help them achieve their goals.

Essential Job Functions

• Working 1:1 with an individual
• Assists with the implementation and completion of orientation plans for person entering, transferring to, or re-entering into agency services
• Assists with the development of the Individual Service Plan and the creation of goals and objectives
• Administers tools that evaluate performance (i.e. observations, vocational and skill assessments, interest inventories)
• Implements vocational plans and accurately documents progress and areas of concern
• Implements behavioral support plans and documents progress
• Provides information to be included in progress summaries/reports which becomes part of the formal case record
• Plans for and provides interesting and challenging activities (in alignment with the individual’s plan) during downtime and free time
• Reports all incidents/accidents to supervisors and rehab counselor timely and conveys accurate and required information
• Prepares the work/activities to be completed by the individual(s) and for monitoring flow from setup to task completion
• Ensures that quality control procedures are followed and corrects/reports deficiencies immediately
• May calculate and document production rates in a timely and accurate manner for payroll and for workmeasurement (time studies)
• Must be alert to potentially dangerous situations that may cause injury
• Must know and follow all health and safety requirements of self, co-workers, and persons served under his/her supervision
• Understands and maintains the required level of supervision in accordance with the individual service plan and/or as instructed by supervisor
• Monitors persons served during work and free time and provides guidance, direction and intervention as needed
• Provides crisis intervention and approved psychological and physical management techniques (PMT) guidance as necessary
• Knows and protects the human rights of consumers
• May have to attend to personal care issues of others
• Attends to site safety concerns (i.e. removes/reports hazards and issues)
• Must transport individuals to and from assigned activities in a safe manner and ensure that all passengers are properly secured
• Must have and maintain clean, valid Public Service Drivers License in accordance with agency policyand state law
• Adheres to program specific procedures regarding vehicle maintenance and care
• Perform any other duties as assigned
