With almost 900 physicians on staff, and a level II Trauma Center, we care for well over 100,000 patients annually across the south shore of Long Island. The Bariatric Dietitian assesses the Bariatric patient’s nutrition status both pre and post-surgery and provides the appropriate nutrition care in collaboration with the health care team. Develops appropriate recommendations for nutrition intervention; documents assessments and reassessments, and associated care plans in the electronic medical record. Provides nutrition education to the patient, family member(s) or caregiver(s). Communicates with interdisciplinary team members including physicians, MLP’s, and nursing staff to coordinate patient care. Participates with community and program events as requested. Performs other related duties and tasks as assigned. Responsibilities: • Employee aids in the orientation and training of new employees. • Addresses and assists in making reasonable accommodation for the special needs of each group served specific to age, language, and disability. • Is able to determine appropriateness of therapy and recommends in the Nutrition Care Process Assessment Form or Progress Note. Contacts physicians to clarify or modify orders when necessary. • Employee demonstrates knowledge of Bariatric surgical procedures, pre and post-surgery guidelines for diet progression and supplementation, ways to promote effective nutrition and life skill factors to support long-term weight loss goals. • Able to provide verbal and written educational material to patients when appropriate. • Participates in clinical education of nutrition interns from clinical affiliates. • Supervises and aids the interns, and nutrition students as assigned. • Participates in performance improvement activities of the department. • Demonstrates verbal and written communication skills when performing Nutrition Care Process (NCP) assessments. • Is able to utilize the hospital information Management Systems (EPIC). • Demonstrates knowledge of drug and nutrient interaction. Requirements: • Bachelor Degree in Nutrition/Dietetics required, Master Degree, preferred. • Must be a Registered Dietitian in good standing as credentialed by registered the CDR. NYS- CDN required within one year of start date. • Computer skills, required. Hospital Information Management Systems (EPIC) knowledge, preferred. Must be flexible with work assignments and able to work in a labor intensive work environment. Demonstrate excellent written and verbal communication skills, professionalism, customer-service focused. Be a self-starter, and possess good time management skills Bilingual, Spanish-speaking preferred. • Has the knowledge base to perform a nutrition care process assessment. • Has the knowledge base to formulate a nutrition care plan (Goals & Interventions). • Be a Registered Dietitian with a minimum of 3+ years acute care experience, Bariatric nutrition experience preferred.
