Develop and implement all aspects of staff action plans. Ensure timely and accurate billing for all services provided. Provide instruction and any necessary supports, adaptations required for each individual.
ESSENTIAL FUNCTIONS/RESPONSIBILITIES:
Attend IAM/Life Plan/Person Centered meetings as needed.
Promote and coordinate positive relationships and open communication with individuals, co-workers, management and any supports associated with the individual in an effective and timely manner.
Develop and implement meaningful activities; activities that promote independence, individualism, inclusion, and productivity. Provide routine supervision of the delivery of these services/supports to ensure proper implementation.
Assist with development of personalized staff action plans.
Acquire and maintain a working knowledge of all individual permanent files including obtaining or completing current documentation.
Facilitate discussions, model and actively participate in activities which are meaningful, age and skill level appropriate.
Identify need and provide support to individual as necessary.
Collect and monitor paperwork to ensure efficient confidential comprehensive record keeping is maintained.
Train newly hired staff on policy and procedure of program to acclimate to individuals served. Mentor and provide instruction to newly hired or assigned co-workers.
Attend and participates in general staff meeting, in-service trainings and other pertinent agency and community program meetings.
Meets and maintains approved driver status and provides safe transportation to individual utilizing an agency van.
Coordinates and develops opportunities for community inclusion and integration.
Maintain professionalism.
Complete monthly summaries.
Support Team Manager as needed.
Maintain a clean, safe and usable space.
Maintain certifications required by agency policies.
Other duties as assigned.
