Responsibilities:
Coordinates and facilitates patient care progression throughout the continuum.
Collaborates with Social Work on the facilitation of transfers internally to SNF, ARF, LTACH and Home Care.
Works collaboratively and maintains active communication with physicians, nursing, and other members of the interdisciplinary team in order to effect timely and appropriate patient management.
Assists in the development of departmental practices and procedures for transitional care planning ensuring compliance and Medicare regulations are met.
Analyzes productivity and other outcome measures.
Identifies opportunities for continuous improvement.
Manages information systems to maximize the efficiency of review and transitional care planning.
Develops and monitors routine data quality validation.
Acts as a liaison with other review-related and transitional care planning related functions and communicates with departments as necessary to ensure coordination of related processes and functions.
Follows Hospital and department policies and procedures with special attention to attendance and punctuality, confidentiality, dress code and display of ID badge, and safety.
Completes mandatory education and training in order to maintain organization and department-specific competencies and requirements.

Education:
Bachelors in Nursing required.
Masters degree in Nursing or related field preferred.
Certifications:
Work Experience:
Minimum two years recent experience in care management or transitional care planning.
At least three years of experience in hospital based nursing.
Previous supervisory experience required preferably in the field of care management.
Physical Requirements:
Works in an environment where there is some discomfort due to dust, noise, temperature.
Requires extensive walking, telephone use, bending, reaching, and lifting items weighing twenty pounds or less.
Must be able to work well under pressure and maintain professional demeanor under adverse conditions.
Extensive reading, writing, use of pagers, fax machines, computers, and copiers.
Sedentary Work – Exerting up to 10 pounds of force occasionally (Occasionally: active or condition exists up to 1/3of the time) and/or a negligible amount of force frequently (Frequently: activity or condition exists from 1/3 to 2/3 of the time) to lift, carry, push, pull, or otherwise move objects, including the human body.
