Market Medical Director – Physician
The CommuniCare Family of Companies currently owns/manage numerous World-Class Nursing and Rehabilitation Centers, Specialty Care Centers and Assisted Living Communities throughout Ohio, Indiana, West Virginia, Virginia, Missouri, Pennsylvania and Maryland. Since 1984, CommuniCare Health Services has provided superior, comprehensive management services for the development and management of adult living communities.
CommuniCare is now recruiting to fill the key position of Medical Director for our newly formed Medical Practice. We are looking for a physician with Long Term Care experience to provide consistent, high quality care to our residents in the Cincinnati, OH market.
As Medical Director, you will be part of an exciting new model of care combining good old fashioned bedside manner with utilizing innovative technologies to dramatically reduce patients’ return to hospital and achieve better outcomes.
Job Duties and Responsibilities (partial listing):
Market Medical Director will have Responsibilities to the Communicare Divisional Team in Ohio.
Market Medical Director will provide support to Communicare’s Medical Practice, Personalized Health Partners (PHP). PHP is a Medical Practice comprised predominately of NP’s as well as Medical Directors.
Market Medical Director may have building Medical Directorship in selected buildings.
Assume the administrative authority, responsibility, and accountability of implementing our medical services, policies, and procedures according to federal, state and local regulations; and current practice of post-acute and long-term care medicine.
Coordinate medical care, including emergency treatment procedures, maintain effective liaison with attending physicians, and implement methods to keep the quality of care under constant surveillance.
Provide necessary physician services for those residents within the skilled nursing facilities and/or clinic environment

Qualifications:
M.D. or D.O. required
Gerontology certification preferred
MD or DO license in State of Ohio
ABMS Board Certified in: Internal Medicine, Family Practice, Geriatrics or Hospice/ Palliative
5+ years of experience working with geriatric or long-term care population
Demonstrated experience working with and educating NP’s
Preferred Qualifications:
Health care management/leadership experience
Familiarity with CMS, Medicare Advantage, SNP Rules and Regulations
HCC and Risk Adjusted reimbursement methodology
Inpatient/SNF utilization metrics and bed day management
State specific collaborative practice agreements and midlevel practitioner licensing
Knowledge of EMR
