Immediate Opening – Outreach Associate:
Community Health & Wellness Partners has an immediate need for a full-time Outreach Associate. All employees at Community Health& Wellness Partners serve the mission of providing quality, whole person, patient-centered medical care to anyone and everyone in our community.
As a Federally-Qualified Health Center, with integrated ancillary services, we are looking for an Outreach Associate to change lives by eliminating barriers and providing a standard of healthcare that improves the well- being of the whole person. Our organization believes that we are about our patients, that we are changing lives in our community, that passion is a part of our core, that we have a strong commitment to outstanding patient care, and we believe our approach to care is raising the bar for providing quality patient care throughout the region. Organizational certifications include NCQA Patient -Centered Medical Home and Diabetes Education Accreditation Program.
Candidate must function as often the first point of contact for all patients utilizing our facilities for health care. Manage the timely and accurate collection of important patient information, and function as a resource to patients entering the facility. Supports and acts as an advocate for patients by assisting them with the registration process. Conducts outreach efforts and implements financial assistance programs of CHWP.
Bilingual in Spanish is helpful, but not required. Associate degree, Medial Assistant certification and/or two-years-related experience helpful, but not required.
