Certified Nursing Assistant/Hospital Clerk (Job ID: 6304)Job SummaryUnder close supervision, assists professional nursing and care providers inproviding care to all patients with respect for their uniqueness. Collaborateswith the RN and healthcare team. Assists with basic patient care andactivities of daily living. Maintains unit specific documentation as required.Orders, maintains and utilizes supplies for the assigned unit.. (EX. Abnormal VS,bleeding, safety concerns, family issues.)2.Assists the RN/LPN in carrying out the established plan of care forassigned patients.3.Functions appropriately in critical situations.4.Participates as a member of the patient care team by asking for additionaltasks, or helping others when not busy.5.Accurately gathers information and performs data entry.6.Performs transportation responsibilities when requested; transportingpatients from one area to another; delivering laboratory specimens, bloodproducts, medications and other hospital/medical items to the requesteddepartments as needed; and includes access to the pneumatic tube system totransport items, including blood products and medications, to and fromthe tube and the desired location.7.Documents Plan of Care making sure information is properly transcribed andcomplete.8.In coordination with the RN/LPN, implements care based on prioritizedpatient needs.9.Participates in a unique, outcome focused plan of care based on currentnursing practice with measurable outcomes and goals for the patient/family.10.Participates with the Coordination of care with multidisciplinary team.11.Participates in the discharge planning process.12.Assists the RN/LPN to identify realistic expected outcomes to facilitatethe patient’s optimum level of health as well as the anticipatedcontinuation of care.13.Works with the RN/LPN to provide a unique, outcome focused plan of carewith measurable outcomes and goals for a patient/family.14.Participates in the peer and coworker review process.15.Collects clinical data to contribute to the Plan of Care in conjunctionwith the RN/LPN.16.Performs chart audits to promote quality improvement.17.Reports response of patient/family or changing condition to the RN/LPNand helps to appropriately update the plan of care.18.Participates in practice changes related to nursing research.19.Practices sound safety and infection control practices according tohospital policy and procedures20.Reports quality, effectiveness and efficiency of nursing care provided.21.Performs quality control on appropriate systems and equipment22.Promotes quality care based upon ongoing monitoring of patient/familyresponse or changing condition23.Provides a safe environment for all patents. Assuring proper patientpositions and patient care throughout the patient’s stay.24.Ensures practice and adheres to professional standards, statutes,regulation,, hospital and departmental policies, and standards of care.25.Assumes accountability for achieving goals set jointly with the manager,educator and charge RN for the next evaluation period.26.Other duties as assigned.*Performs clerical functions such as typing, formatting and enteringcorrespondence and reports into appropriate MS formats; attends departmentalmeetings, takes minutes and transcribes theminto draft and final formsKnowledge, Skills & Abilities1.Ensures all duties, responsibilities and competencies
