Description
Responsible for providing support to providers in-person and virtually to improve the effectiveness and efficiencies of provider practices. Receives guidance and direction in providing operational and administrative support for the implementation, auditing and resolution of loading issues for payment innovation programs.
How you will make an Impact;
Works to advance the Medicaid Transformation efforts through strategic support for Advanced Medical Home (AMH) Tier III practices.
Supports and facilitates value based JOCs (Joint Operating Committee meetings).
Obtains and analyzes quality metrics and reports for care opportunities, supports practice implementation of care coordination and care management, identifies action plans for providers to implement to improve cost, quality and the patient experience and participates in design, development, and implementation of community learning forums.
Collaborates with all Advanced Medical Home (AMH) teams including but not limited to: provider collaboration manager, provider clinical liaison, provider contracting and network relations consultants to meet provider and practice service needs.
Actively participates in developing AMH program recommendations and action plans.
Develops and maintains contact database and disseminates communications to providers as necessary.
Aggregate and analyze provider data form various sources to recommend and implement solutions to assist with provider collaboration activities.
Collaborates with all Advanced Medical Home (AMH) teams including but not limited to: provider collaboration manager, provider clinical liaison, provider contracting and network relations consultants to meet provider and practice service needs.
Actively participates in developing AMH program recommendations and action plans.
Supports the efforts of providers and practice teams related to care delivery interventions resulting in growth towards AMH Tier 3 attestation and ultimately cost of care savings.
Promotes practice transformation towards meeting the goals specified by NC-DHHS.
Minimum Requirements:
Preferred Skills, Capabilities and Experiences:
Public health environment, health policy, practice management experience strongly preferred
Experience with value based and/or health care contracting strongly preferred
Claims and capitation knowledge
Knowledge of Care Management
May require up to 25% travel
Previous Medicaid experience preferred.
Local County Department of Human Services experience preferred.
