Typical Duties:
+ Carries a caseload of 60 high risk pregnant clients which is representative of the case contracted to the agency by IDHS.
+ Complete monthly one-on-one face-to-face/month; Home Visit (HV) each trimester or more often as indicated by risk assessment as designated by program mandates and according to professional judgments. One contact every month via the Phone. Average of 20 face to face a week.
+ Performs intensive prenatal case management service involving the application of social work principles and theory in servicing high risk clients within the IDHS Guidelines.
+ Complete a comprehensive in-home assessment of assigned cases within the guidelines established by BBO Protocol.
+ Perform a variety of case management duties, such as, conducting interviews with clients and other appropriate persons for purposes of formulating treatment plans for cases of high complexity.
+ Perform outreach activities such as contacting OBGYN’s services providers, schools, etc. to locate and enrolled eligible clients into the program.
+ Develop a written service plan for each assigned cases. Utilizes basic case management methods and techniques to achieve effective treatment goals.
+ Increases analytical and psycho/social diagnostic skills and resourcefulness in developing case management processes.
+ Follow up and document as appropriate on referral given to client.
+ Coordination of care to ensure we meet client needs.
+ Contact other social service agencies regarding client situations, for the purpose of making inter-agency referrals.
+ Provide clinical reviews of high risk cases receiving intensive prenatal case management services
+ Provide appropriate referral to clients in accordance with specified referral procedures.
+ Assist client in utilizing all available resources in the area.
+ Report progress on caseload to the supervisor as well as in the Cornerstone Database System.
+ Perform record keeping functions as required by agency.
+ Attend professional development and training sessions within and outside of the agency as well as other local professional workshops to enhance job skills and understanding of social service programs.
+ Perform other tasks as assigned by the supervisor.
