We are accepting applications for a Contact Investigator/Contact Tracer. This position is responsible for reaching out to people with laboratory confirmed, probable, and suspected disease transmission during the public health emergency of COVID-19.

Essential Functions include:

+ Conduct interviews with patients to gather history, source of illness, list of close contacts, and activity history to all persons who have been in close contact with a COVID-19 infected individual.

+ Provide education, information, and guidance to interrupt the ongoing transmission and to reduce the spread of the virus.

+ Obtain symptom information from contacts.

+ Provide information on quarantine and isolation procedures.

+ Make referrals to testing, clinical services, and other essential support services

+ Work closely with public health staff to effectively complete all case investigations.

+ Systemically collect and record all information given.

+ Maintain confidentiality in relation to HIPAA and information received.

+ Good use of motivational interviewing techniques.

+ Good use of prioritization, prompt action, attention to detail in documentation, and data management.

+ Notify or verify receipt of a positive laboratory report or probable or suspected diagnosis of COVID-19.

+ Ensure daily monitoring of patients during isolation and quarantine of positive case and close contacts.

+ Accurate and complete data entry into State’s communicable disease system.
