Harry Hynes Memorial Hospice was founded by community volunteers in 1983 as a nonprofit organization, we are honored to be a leading provider of Hospice and Palliative care in South Central and Southeast Kansas. Having served more than 40,000 patients and family members, we are experts in specialized care designed to provide support to you and your loved ones during an advanced illness. Our team of professionals, including a medical director, nurses, hospice aides, social workers, and volunteers, work in coordination with the patient’s physician to provide pain and symptom management, as well as social and spiritual support.
Summary
Coordinate the drug distribution and clinical pharmacy activities for nursing and support departments with the hospice. Responsible for safe and efficient methods for coordination of established pharmacy policy concerning drug patient care. Provide clinical drug information to meet the needs of the patient, nurse and physician. Responsible for meeting medication specific COPs.
Responsibilities

+ Reviews and interprets physicians’ orders for appropriateness and effectiveness, side effects or potential side effects or adverse reactions, drug interactions with other medications, prescription, herbal, over-the-counter, cost effectiveness of medications ordered, duplication of orders, appropriate coverage of medications, evaluation of medications associated with lab monitoring.
+ Dispense prescriptions to outpatients in accordance with hospice policy and in conformance with regulations and laws dealing with the class of prescription and maintain an accurate up-to-date patient profile.
+ Maintain necessary controls with respect to audited controlled drugs
+ Provide drug information such as dosing, compatibilities, formulary substitutes, etc. to nursing, physicians and other health care practitioners.
+ Prepare specialized solutions for administration via parenteral route and ensure that the stability and sterility of these solutions is appropriate.
+ Participate in in-service education programs when requested.
+ Arrange to obtain drugs in an emergency.
+ Maintain a positive, professional, and instructional interaction with all staff. Verify the work performed by supportive personnel, which includes, but is not limited to, patient drug data entry, cassette filling, prescription filling, drug order, and all necessary reports/paperwork.
+ Maintain up-to-date knowledge of the profession by participation in organization meetings, in-services, reading journals; comply with National Standards of Continuing Education to maintain professional competency and proficiency
+ Participate in clinical programs instituted in the department such as drug level monitoring, comprehensive medication profile reviews at sign on and bi-weekly at IDT, auto-substitutions for specific medications.
+ Participate in special studies as requested.
+ Assure that the quality of professional services is maintained.
+ Perform other duties as requested.

Qualifications, Education and Experience
+ Current valid Driver’s License and current auto insurance
+ Must be a Licensed Registered Pharmacist in the state of Kansas.
+ Acute care experience a plus.
