Position Summary
The Clinical Care Coordinator (LPN) assists the Clinical Manager in supervision the home health services to patients in their place of residence in accordance with physician orders. The Clinical Care Coordinator (LPN) identifies/organizes/leads in-service programs for field clinicians, conducts clinical orientation, performs audits and provides clinical support and direction to office and field teams.
