Hope Hospice, Inc Hiring for Medical Records/ Coding – Temporary Job at Dublin, CA Full Time

Responsibilities:

Responsible for the generation and maintenance of the patient medical records. Maintains all medical records with current information and physicians order verification, according to agency policy and all applicable Federal and State regulations. Provides timely review of clinical data and assigns the accurate International Classification of Disease (ICD-10-CM) and current procedural terminology to patient records while complying with the regulatory requirements of the Federal Government, State licensing agencies and with Hope Hospice policies and procedures.

Essential Functions:

• Responsible for all patient medical records, assuring that all components are filed and present in the patient chart at all times.
• Responsible for opening and closing of patient medical records, including ensuring the completion of any termination agreements or forms and chart audits when patients are discharged.
• Ensures physician signatures are in place on filed documents for open and closed charts to meet the agency regulatory guidelines.
• Manage calls the pertain to the patient medical records
• Manage and track receipt of Plans of Treatment (POT) and Physician Interim Orders (PIO)
• Responsible for and mailing and tracking of discharge summaries and re-certification forms to physicians
• Alerts appropriate management team members regarding late or missing documents required for medical records.
• Reviews and interprets information in the medical record and assigns the correct codes utilizing ICD codes and or CPT codes to the diagnoses/procedures according to the coding guidelines.
• Maintains knowledge of and complies with established policies and procedures governed through Medicare including private insurance.
• Protects the confidentiality and privacy of patient information.
• Collaborates with billing in regard to insurance, certification dates, and ICD coding.

Required Experience

• Minimum 2 years’ experience in a medical setting.

Education / Licenses

• Certified Medical Administrative Assistant certification preferred

Pay: $20.00 – $25.00 per hour
