Job Description/Essential Functions
Horizon House, a non-profit organization, has been widely recognized for its services as a provider of community-based behavioral health, intellectual disabled and homeless services in Eastern Pennsylvania and the state of Delaware. Each year, Horizon House provides a continuum of services at over 100 sites to over 4,5000 adults with psychiatric or developmental disabilities, drug and alcohol addictions, and/or homelessness. A pioneer in the field of community-based rehabilitation, Horizon House is widely recognized for its innovative approach to the delivery of services in a community setting.
We are seeking a Full Time Behavior Service Coordinator to work in our Developmental Services Division in East Norriton, PA.
Some of the responsibilities include the following
To coordinate internal and external behavior specialist services to ensure quality training on individual service plans.
Provide systematic and individualized behavior support in compliance with the Agency’s overall purpose and the individual program goals and objectives.
To develop positive support strategies, plans, and data analysis for people with challenging behaviors.
To provide oversight, counseling and training to staff in areas related to the individual’s plans.
Provides direct supervision and ensures training to Care Managers
Provides clinical supervision to Social Work interns and practicum students
Job Qualifications
M.A. Degree in Human Services/Social Sciences/Healthcare or a related field and at least 2 years of experience in the ID field required 2 years in the MH field a plus.
Very good communication skills prior experience in writing programming goals and behavioral plans.
Previous clinical experience conducting group and/or individual therapy sessions required.
Strong analytical and program development skills.
Ability to work independently.
Detail oriented and strong organizational skills.
Experience writing functional analyses and interpret data.
Possession of a valid driver’s license with an acceptable driving record.
PC proficient MS Word, Power Point, and email touch typing at 45 wpm.
License required within clinical/social worker field. LPC, LSW, LCSW.
Scope
Frequent talking and listening.
Requires the ability to lift, carry, fold, kneel, reach, stack, stoop and bend which may include direct intervention with participants.
Must be available for local and regional travel, using own vehicle.
Possess manual dexterity and fine motor skills.
May require on-call crisis intervention to intervene or counsel staff and/or participants through emergency situations.
May be required to attend training to become a certified agency investigator. May conduct investigations and participate in the agency rotation of on-call investigators. This requires carrying a cell phone and responding to investigation hotline calls
Requires carrying a mobile phone during regularly scheduled working hours and scheduled on-call rotations.
May be required to attend training to become a personal outcomes interviewer for the division.
