Intandem. 1487 Sherwood Ave INT N UNION OVERNIGHT RESIDENTIAL AIDE MAIN FUNCTION: The Direct Support Professionals/Residential 2 (Overnight Residential Aide) functions as an adjunctive member of the Interdisciplinary Team (ITT), participating in and providing input into the formulation of the individual resident’s program plan. The Direct Support Professionals/ Residential 2 (Overnight Residential Aide) provides direct care to individuals primarily during the night hours and is responsible to provide necessary support and monitoring of individuals to ensure their safety and well being at all times in accordance with their HCBS Waiver plan of services and establishes policies and procedures. DUTIES AND RESPONSIBILITIES: 1. Implements the residential habilitation plans, nursing care plans and protective oversight plans as prescribed, including chart of intervention with individuals. 2. Provides assistance in and performs as indicated routine program activities, such as personal care needs, meal preparation, housekeeping, and light maintenance. 3. Assists support staff with passing medications to individuals as needed. 4. Successfully completes and maintains medication training and other training mandated by OPWDD if applicable. 5. Acts as communication linkage via performance of routine office related activities (i.e., telephone calls) with other staff members. 6. Performs clerical duties as related to maintenance of individual files. Reads and legibly documents in the daily log all important and/or significant events, which took place during tour of duty. 7. Provides general clean up of the house, according to specific instructions for each residence. General clean-up includes, but is not limited to, dusting furniture, emptying garbage, sweeping and mopping kitchen floor, vacuuming, and laundering nightly. In addition, any extra cleaning duties as specified by supervisor. 8. Assists individuals with personal hygiene, feeding, dressing, and all areas of activities of daily living. 9. Must attend any meetings or in-services as designated by supervisor. 10. Position requires stay awake responsibilities — sleeping is not permitted. 11. Ensures that individuals and their correspondents are treated with the utmost dignity and respect and allows individuals to fully exercise their human rights. 12. Deliver and document services delivered in the format required by regulation and Agency/Program Policy & Procedure. 13. Take attendance specific to individual participation in services on a nightly basis. 14. Must comply with the Agency and Program time and attendance policies including call-in protocol. 15. Responsible to comply with the Agency Corporate Compliance Plan and Code of Ethics. 16. Responsible to report any suspected violations of the Agency Corporate Compliance Plan or Code of Ethics to immediate supervisor, program administrator or Agency’s Corporate Compliance Officer. 17. Accepts Agency/Program safety procedures and establish safe work practices in day-to-day functions of the job. 18. Maintains confidentiality of individual information in accordance with the requirements of NYS Mental Hygiene law and the Agency’s HIPAA Privacy Policies & Procedures. QUALIFICATIONS: Education/Experience: High School Diploma/GED with one (1) years Human Services experience preferred. Essential Requirements: * Must complete and maintain certification in Medication Administration. * Must complete and maintain certification in SCIP-R at the level required to meet program or site specific needs. * Must be able to perform lifting techniques required per program or site specific needs with no restrictions. Preferred: Possession of a valid New York State driver’s license and satisfaction of the Agency’s Driving Policy, which includes a satisfactory driver’s license record (MVR).
