Position Description
Reporting to the Clinical Manager, the Care Coordinator is responsible for providing therapeutic intervention and care coordination to children and youth participating in the Binesi Waziswan Healing and
Treatment Services at Kenora Chiefs Advisory. The Care Coordinator will hold a caseload of clients participating in voluntary residential treatment, supporting them before, during and after to ensure seamless transitions to and from the program. They will create and oversee wellness plans that encompass the mental, emotional, physical and spiritual needs and goals of the child/youth. The Care Coordinator will ensure that all clients will have access to the services they need, including mental health, education, primary care, cultural and land-based, as well as other supports identified by the child/youth and family.
The Care Coordinator will work collaboratively with a team of providers to ensure the success of children/youth participating in the program.

Reports to: Director-Binesi Waziswan Healing and Treatment Services
Summary of Duties
➢ Work with the care team, client and family/caregiver to develop and coordinate wellness plans that encompass spiritual, mental, emotional and physical wellness;
➢ Ensure that clients have access to the services they need including mental health, education, primary care, cultural and land-based, as well as other supports identified by the child/youth and family;
➢ Support clients while waiting for treatment, during treatment and post-treatment to ensure smooth transitions;
➢ Ensure referral packages have been completed in full by supporting clients and families, as needed;
➢ Hold case conferences with mutual providers and families/caregivers to ensure communication across the care team;
➢ Monitor clients’ progress, goals and needs throughout the course of treatment;
➢ Facilitate connections to appropriate culture-based interventions, as needed;
➢ Develop discharge plans by making referrals and coordinating follow-up services to ensure a warm hand-off;
➢ Identify and provide support to families upon discharge of the child/youth, including psychoeducation, referrals to local services, and supporting the child/youth to continue at home with the goals and skills developed in the program;
➢ Ensure all relevant documentation is accurate and completed in a timely manner, including case notes, intake packages and discharge plans;
➢ Adhere to KCA’s policies and procedures, in accordance to the KCA Code of Ethics;
➢ Consult the Management Team for supervision;
➢ Engage in ongoing professional development and trainings.
Qualifications
➢ Minimum 1-year experience in the mental health field working with children, youth and/or families;
➢ Care coordination or case management experience is an asset;
➢ Excellent rapport and ability to work with youth;
➢ High degree of organization and ability to coordinate appointments, applications, referrals, etc.;
➢ Ability to develop strong working relationships with service providers across sectors;
➢ Excellent skills in the following areas: teamwork, computer, networking, interpersonal, presentation and communication (written & oral);
➢ Knowledge of the Anishinaabe culture and language, and experience working in First Nation communities is an asset;
➢ Ability to demonstrate and work through a lens of sensitivity to diversity related to race, religion, culture, cognitive functioning, poverty, sexual orientation and gender identity.
Education:
➢ Degree or diploma from a recognized post-secondary institution in the field of social work, social services, psychology, child and youth studies or a related field.
Position Requirements:
➢ Valid driver’s license and vehicle;
➢ Willing to travel (when required) to any Treaty 3 First Nation communities;
➢ Current satisfactory criminal reference check with vulnerable sector search;
➢ Understanding of the language of the Anishinaabe language (Ojibway) is an asset.
