Job Description
The Perioperative Surgical Home (PSH) is a care coordination program that serves patients having surgical or interventional procedures at LG Health facilities. The PSH advocates for the surgical patient by collaborating with providers on identified issues or concerns from the time the decision for surgery is made until the patient returns home. The multidisciplinary PSH team has the capacity to do the legwork necessary to ensure a comprehensive patient centered plan is in place prior to the patient presenting at one of LG Health’s facilities for a surgery or invasive procedure. Our goals are to contribute to improving patient outcomes and experiences, costs, and focusing on patient education and advocacy.
Penn Medicine LG Health is seeking an experienced Nurse Practitioner for the Perioperative Surgical Home (PSH) team. The Nurse Practitioners are an integral part of the PSH team. They work collaboratively with all PSH providers (Administrative Assistants, OR Schedulers, RNs and Anesthesiologists) on a daily basis as well as with other providers and customers beyond the PSH.
Hours: The position would include 32 hours per week, with hours primarily between 0700 and 1700 Monday through Friday. No on call, weekends or holidays currently.
JOB REQUIREMENTS
MINIMUM REQUIRED QUALIFICATIONS:
Valid Certified Registered Nurse Practitioner license and Certificate of Prescriptive Authority in Pennsylvania
Valid Pennsylvania Registered Nurse licensure
Certification by ANCC or other qualifying agency in area of specialty required.
Related experience/clinical training (4-5 years NP experience in Family Medicine, Med/Surg, Emergency Medicine/Urgent Care would be preferred).
