Liberty Healthcare is currently hiring LICSWs for part-time and full-time “Assessor” positions. These are field-based positions which requires regular daily travel to client homes throughout the DC Metro area.
Liberty Healthcare operates a program in the District of Columbia which serves people who are applying for long term care supports and services. The goal of this program is to enable people to receive the support services they need to live as independently as possible – while applying a valid, reliable and quality assessment process to prevent fraud, waste and abuse. Specific services include intake, applicant eligibility assessment, level of care determinations, recommending a range of service hours, and linking beneficiaries to home health providers.
Your responsibility as an Assessor is to assess client health and environmental factors to help determine their eligibility for services. This role is limited to conducting assessments and does not involve direct client care.
Specific responsibilities will include assessing client’s activities of daily living and evaluating client’s home environment – and completing electronic reports related to client’s health status, diagnoses, medications and other pertinent factors.
You have the option to work a traditional full time Monday through Friday schedule – or a flexible part time schedule.
As an Assessor and full-time employee, you can expect and annual salary of $89,000 and comprehensive benefits package. Compensation for part-time employees is prorated. Assessors are also reimbursed for approved travel expenses.
You’re a good fit for this position if you possess the following:
Professionally licensed as a LICSW in the District of Columbia
Experience in utilization review, case management, service coordination, MLTSS or LTSS
Ability to effectively use a laptop and other remote office technology
Willingness to travel daily throughout the District to conduct in-person client assessments in homes
