Our Transition Team is a supplemental care unitof skilled nurses whose primary function is to quickly and efficiently getcases home while primary nursing staff are actively recruited. The team alsosupports the local office by covering missed opportunities.
How it works:
Once the referral is received, we immediately beginsourcing full-time staff. Prior to discharge from the facility, patients willmeet with an internal office representative and a lead nurse form thetransition team to detail how the program works. Once the patient transitionshome, we’ll employ our nurses to ensure full coverage until the permanentnursing staff can be identified.
Transition Team nurses provide care for all authorizedhours in a home during a provisional period between 1-90 days. during thistime, our office staff will be recruiting to identify full-time,replacement home healthcare nurses. The new caregivers will have an opportunityto train and shadow in the home with the transition team caregivers prior toworking independently with the patient. Once the replacement nurses are trainedand have a schedule, the Transitions Team are then phased out of the home andonto the next. Not just anyone can be a transition team nurse. You are the faceof the company. You are the first person the family thinks of when they are newto homecare. You make a difference. This is a rare opportunity!
