Position description
The TCU RN Coordinator assesses and reviews all referrals for transitional care within the SWMN Region for appropriateness for placement. The TCU RN Coordinator completes nursing assessments on referred patients and collaborates with social work in the Central Referral Office for appropriate placement locations. The TCU RN Coordinator coordinates with Critical Access Hospitals in the SWMN to complete handoff of patient assessment information and patient needs to determine if sites are able to accept the patient and then completes facilitation of handoff of information from the hub hospital to the CAH site accepting the patient.
Qualifications
Experience: A minimum of three to five years of hospital or clinical experience required. Population specific experience preferred.
Education: Graduate of a baccalaureate National League for Nursing (NLN) or Commission on Collegiate Nursing Education (CCNE) accredited nursing program with three to five years of experience. Will consider AD RN applicants meeting all other qualifications who commit to baccalaureate degree completion within six years.

Special Skills: Knowledge of population characteristics and desired outcomes Excellent communication skills (verbal and written) Experience working in a team environment Computer skills (e.g. PowerPoint, Excel, Word) required, prior experience with electronic medical record systems preferred Ability to work flexible hours, which may include days, limited weekend hours may be required. Ability to adapt to unpredictable situations within the work setting Demonstrated leadership, effective communicator, and excellent critical thinking skills Demonstrated ability to use data for trend analysis and decision making. Previous experience in a critical access hospital setting preferred.
License or certification
Licensure & Certification: Current RN license by applicable state requirements. Maintains Basic Life Support (BLS) competency. Additional specialty certification/Training: specific to care coordination as required by the work area.

Compensation Detail
Education, experience and tenure may be considered along with internal equity when job offers are extended. The minimum salary every 2 weeks is approx $2,750.40, based on a full-time position.
