Duties and Responsibilities
The incumbent is part of a global public health organization and is expected to support the Florida Department of Health in Pinellas County in its mission to protect, promote and improve the health of all people in Florida through integrated state, county and community efforts and its vision To be the Healthiest State in the Nation. Furthermore, the incumbent is expected to incorporate the agency values of Innovation, Collaboration, Accountability, Responsiveness and Excellence in all aspects of their work.
Meets or exceeds SMART performance expectations to include but not limited to: DOH-Pinellas snapshot measures, Comprehensive Program Communications, Quality Assurance reviews, Supervisory Ride Alongs, DOH-Pinellas Core standards, PMC and other high level meeting attendance, Presentations as needed and Budget/Fiscal Management
This is a highly specialized position involving knowledge of Linkage to Care principles ensuring that individuals infected with HIV are linked to care. Linkage Coordinators work with clients to remove potential or present barriers to medical care. The Linkage Coordinator is responsible for linking recently diagnosed individuals, or those that have fallen out of care, into primary medical Care and HIV care services. This position will work with local Health Services Representatives and HIV surveillance staff, HIV medical care providers, community-based organizations and other key stakeholders to identify individuals recently diagnosed with HIV or those who have fallen out of care. Once these individuals have been identified, it will be the responsibility of this position to link these indviduals to HIV medical care within 90 days of diagnosis, or 90 days of notification that they have fallen out of care.
Works collaboratively with Health Service Representatives, or other appropriate staff, to establish contact with client following postiive confirmatory HIV test result; investigates and documents the preferred meothods to communicate with client and secures consent forms authorizing communication between the client and department staff, makes at 3 documented attempts to confirm that client attended medical appointment; if client did not attend, determines why and works with client to remove barrier to care, continues to work with client until they attend medical appointment and are engaged in medical care.
Using available databases (CAREWare, ADAP, HMS, PRISM, etc..) identifies HIV+ individuals that are not in medical care, not taking antiretroviral (ARV) medications, or have tested positive for HIV but have not been notified of their status; contacts individuals that are not in medical care, taking ARVs, or that have tested HIV+ and not been notified of their status; works with local HIV medical care providers and AIDS service organizations to identify and notify clients that are out of care from their client databases; makes at least 3 documented attempts to schedule an appointment with a medical provider with the client; makes at least 3 documented attempts to confirm that client attended medical appointment; if client did not attend, determines why and works with client to remove that barrier to care; continues to work with client until they attend medical appointment and are engaged in medical care
Coordinates with members of treatment team to enhance linkage to care, engagement and improve data collection and tracking; ensures services are not duplicated and all clients are linked to care and/or have a follow-up plan, maintains records and other work material in a clear and organized manner, according to agency guidelines and/or programmatic requirements; adheres to confidentiality protocols and privacy of persons, documents, data and communications; maintains a current list of available local resources to facilitate clients navigating the system of care.
Provides back up to other members of the Division of Disease Control and Health Protection as assigned to an operations level. This may include but is not limited to: Direct Observed Therapy (DOT), outreach efforts, immunization clinics, disease investigations and traveler monitoring.
Knowledge, skills and abilities, including utilization of equipment, required for the position:
Ability to engage and work effectively with diverse client populations
Ability to work independently and efficiently and use problem-solving techniques
Must possess HIV/AIDS/STD knowledge, including prevention, transmission, and treatment
Ability to plan, organize and coordinate work assignments.
Ability to communicate effectively, both orally and in written format.
Ability to establish and maintain effective working relationships with others.
Working Hours: 8:00am to 5:00pm Monday-Friday
