Job Requirements
Education
Bachelors of Science in Nursing (BSN) from an accredited school of nursing required by December 31, 2020 if hired after 2012 or five years from hire date for nurses hired after 2015.
Successful completion of annual mandatory education requirements.
Graduate of an accredited school of nursing required.
Graduate of a Wound Ostomy And Continence Nurse (WOCN) accredited Enterostomal Therapy (ET) nursing program or willingness and ability to pursue the wound portion of the accreditation program within one year of hire required.
Experience
A minimum of two years of nursing experience in related speciality area required.
License/Certifications
Current valid Nebraska Registered Nurse (RN) License, valid compact multistate license to work in Nebraska, or a temporary permit while awaiting licensure required.
Current American Heart Association Basic Life Support (BLS) Certification required.
Certification(s) in area of specialty preferred.
From National Certification Corporation (NCC) within 1 year of eligibility.
Certified Wound Ostomy Continence Nurse (CWOCN) preferred

Strong interpersonal skills necessary to effectively communicate and interact with patients, family members, hospital personnel, and co-workers.
Ability to work independently and with excellent organizational skills required.
Ability to multi-task and prioritize work assignments.

Essential Functions I
Provide appropriate care specific to the age of the customer/patient, to ensure understanding and comfort level of treatment as outlined in the Population Specific Criteria.
Demonstrate ability to identify opportunities for and give individualized customer service/patient care according to customer/patient needs.
Perform an initial and ongoing assessment to determine the assigned patient’s ongoing nursing care needs.
Include data from the patient/significant other(s) in the assessment as appropriate.
Prioritize data collection according to patient’s immediate needs.
Use assessment findings in discharge planning.
Develop/review/revise and individualize the plan of care to address the patient’s nursing care needs for optimum patient care outcomes. (Consultation and resources, nurses as teachers.)

Evaluate patient care to determine response to care/interventions for optimal patient outcome.
Monitor outcomes and documents patient response to interventions.
Evaluate the patient’s capabilities and/or limitations related to nursing and medical interventions.
Continually monitors the patient and/or significant other(s) ability to manage care needs after dismissal.
Give a complete verbal or written change of shift report to the oncoming care provider. Includes current care requirements, to include SBAR (situation, background, assessment and recommendation) content, and is familiar with the overall plan of care.
Recognize and address patient and family capabilities and limitations (physical and mental).
Perform technical skills competently and independently to assure patient safety (autonomy).

Complete appropriate report form for any risk (e.g., variance/incident report, patient concern report, maintenance request).
Communicate to appropriate personnel regarding unresolved issues.
Work within the scope of practice as defined by State Nurse Practice Act.
Observes appropriate precautions when administering hazardous medications or handling excreta from patients who have received hazardous medications.
Review, delegate and evaluate plans for appropriate utilization of staff at all levels of practice in accordance with the provision of the state’s Nurse Practice Act and the Professional Standards of Practice.

Performs an evaluation on initial visit of patients. Establishes appropriate treatment plan from initial evaluation.
Review medical records to assist in the evaluation process and contacts attending physician(s) to gather information as needed.
Coordinates care of patient between WOC services, referring physician(s), and floor nursing staff as necessary.
Establish and implement a treatment program with regular follow-up to ensure efficiency of treatment, by basing treatment on objective evaluations and achievable short and long term goals with time frames, with direction toward desired outcome.
Treatment must be appropriate for the patient and constantly re-evaluate the effectiveness of the plan.
Performs technical skills competently and independently including but not limited to pre-operative marking of stoma site, development of appropriate management systems for patients with wound and skin care needs.
Ensures patient’s progression towards clinical goals.
Content and frequency of the program are adjusted as the patients condition or status warrants.
Treatment plan is appropriate and implemented within a reasonable time frame.
Provides patient education to facilitate recovery/progress.
Education is evidenced by either documentation, written handout or direct observation in 100% of the patient population
When possible, patient will convey understanding of key principles, precautions and instructions by demonstration.
Employee will utilize interpreters, etc., to assist with instruction when needed or requested.
Maintains operational and clinical records to comply with professional standards of practice, Federal and State regulations and established policies/procedures. Provides appropriate patient billing, to ensure accurate billing of services and facilitate clinic operations.
Completes accurate record documentation in various practice settings within appropriate time frame.
Responsible for content and accuracy of records related to WOC nursing practice.
Employee shall be responsible for identifying the appropriate charges based on treatment rendered, 100%.
Directs and participates in cost effective care delivery including distribution of products.
Participates in product evaluation and research acitivities related to practice.
Participates in clinical instruction/orientation of new staff , direct care nurses, and students to facilitate their education and promote the profession and department, as requested.
Provides supervision and guidance appropriate for each new staff member/direct care nurse/student, to ensure appropriate evaluation and treatment of patients as well as educational outcome.
Complies with school / department requirements 100% for supervision / feedback.
Provides in-service programs for the hospital, as well as participation in new employee orientation, to facilitate an active learning environment.
Presenting a staff in-service program within one month of attendance at a professional workshop or seminar.
Attend all mandatory in-services and assist with new employee orientation.
Attend at least two clinical inservices per year.
Assist with program development when indicated, to assist with the development of marketing strategies.
Attends meetings and assisting with development and implementation of programs to achieve goals for clinic and marketing of these programs to physicians, the nursing staff and the public.
Assist with clinic coverage as needed. Fosters clinical excellence by demonstration of current knowledge base and participation with program development.
Essential Functions II
Participate in mandatory in-services and/or CE programs as mandated by policies and procedures/external agencies and as directed by management.
