Discharge Planning Assistant – Social Work – Full Time – Day Shift

Across NewYork-Presbyterian/Weill Cornell Medical Center, the interdisciplinary team approach to care coordination drives our quality outcomes and amazing patient stories. As a Discharge Planning Assistant, you’ll assist with facilitating successful discharge plans which are critical to length of stay reduction and patient flow. Join a team of Care Management, Social Work, and Nursing, in providing a high level of customer service to patients by ensuring a proper and timely discharge. Attend Interdisciplinary rounds and communicate insurance issues with team. Makes appointments with providers for post discharge follow up. Go above and beyond by securing resources for patients, contacting providers and community referrals to obtain authorizations, and setting up placement after discharge.

This shift for this position is 9:00am-5:00pm, Monday – Friday.

Preferred Criteria

Bachelor’s Degree

Required Criteria

High school diploma or GED

1 year of experience in a healthcare setting

At least one year of experience in a healthcare setting is required. Volunteer and internship time is acceptable

Strong interpersonal skills, willingness to learn and ability to work in a fast paced and changing environment

Knowledge of Microsoft Office products and general computer literacy

Excellent communication skills, both oral and written.

Detailed oriented, good organizational skills and ability to multi-task

Ability to work independently
