Summary:

The position will work closely with individual providers and allied health staff in the primary care physician practice. The Coordinator, will work with the Program Manager and providers to evaluate their patient populations who qualify for quality measures endorsed by EMMC’s own population health initiatives. The Coordinator will identify patients who are not meeting quality measures and work with practice staff to ensure the patient is compliant and quality measures are met. The Coordinator will work with other members of the Ambulatory Quality Team and the Practice Team to reach mutual goals for quality of care and patient safety.

Responsibilities:

PEOPLE

1. Demonstrates effective and courteous Customer, Physician/Practice Staff, and other hospital staff communication skills. Answers all calls promptly and courteously. Responds to and resolves issues promptly through effective Communications. Uses appropriate chain of command. Feedback is provided in a constructive manner. SERVICE

1. 100% documented evidence of attendance of staff/department meetings, and/or reading/initializing the minutes, all other memos and postings.

2. Develop, manage, train and audit practice staff to meet clinical metrics and hold an orientation session on quality to all new staff during their first week.

3. Regularly audit all practice quality metrics (examples are ACO, NCQA, PCMH Pilot, Anthem, Humana, etc.). The regularity of these audits will be determined by practice performance, but could be daily, weekly, monthly, quarterly, etc.

4. Runs & manipulates reports (as needed) and sends to practice leadership, Ambulatory Quality Manager and other leadership as designated.

5. Timeline of reports and other duties to be completed by population health coordinators will be audited by Ambulatory Quality Manager.

6. Attend and serve as liaison for practice at Clinical Performance Improvement Meeting.

7. Demonstrates effective leadership skills.

8. Collects, reviews and documents patient’s current health status and history according to standard.

9. Is knowledgeable in preparing patients to be seen in the practice. Documents all aspects of the nursing service according to hospital and practice standards.

10. Performs and documents results of point of care testing and appropriate interventions, EKG’s, visual acuity, audiometry, NST, tympanometry, peak flow, nebulizer treatment, pulse oximetry, wound dressings and ear irrigations are performed and documented per practice guidelines, (after orientation is complete).

11. Follow-up visits are scheduled at time of discharge, per protocol and Patient/Family, etc. is notified of next appointment.

12. Assist physicians with admissions by calling registration, call report to the Unit, photocopy all practice documentation to go with the patient, arrange transport. Completes all paperwork and sends with patient.

13. Accepts telephone orders within job description or delegates when appropriate, documents and reads back.

14. Performs ongoing patient screening, planning and implementation of patient care by effectively implementing the care, and teaching plan in collaboration with provider and clinical leader. .

15. Using Prescription Refill Line, refills routine prescriptions noted on medication list or refers to Clinical Coordinator for triage.

16. Participates in and/or delegates daily maintenance of working environment (e.g., cleaning, stocking of rooms).

17. Coordinates patient educational materials and ensures their availability.

18. Documents patient readiness to learn and barriers to learning.

19. Duties are adequately performed as needed or seeks out supervisor for guidelines.

20. Orders, and inventories all state-supplied immunizations. Ensures proper storage of state immunizations. Ensures proper use of immunization logbook.

21. Participates in meetings/project teams as assigned.

QUALITY

1. 100% compliance of: iCARE (complete annually at least seven days prior evaluation), Employee Updates, and any other in-services/meetings and training as assigned.

2. Job Hazard Analysis: Has received and understands his/her job analysis.

3. Has documented evidence of continuing education in knowledge and appropriate skills.

4. Will work with practice and quality leadership to establish a plan to implement all new or changed processes.

5. Work with quality coordinators to identify deficiencies. System Deficiencies will be analyzed/fixed and/or reported to committees as appropriate. Staff deficiencies will be reviewed and reported to practice management/clinical lead and Ambulatory Quality Manager. If possible team will make recommendations for process improvement.

6. Monitors established baseline quality of care of the practices and providers for certification and pay for performance areas as designated.

7. Works with practices to develop means of improving the quality of care using any and all techniques available to meet certification elements.

8. Maintains certification and BSL certifications. Maintains PPD, and blood glucose certifications.

9. Demonstrates correct technique in the use of equipment, according to product information and policy and procedure manuals.

10. Follows appropriate procedure for obtaining and returning/cleansing/disposing of equipment and supplies.

11. Is responsible for unit cleanliness, maintaining a neat and safe environment.

12. Administers and documents medications, according to nursing policy and procedure following the 5 Rights: 1. Right Patient 2. Right Medication 3. Right Time 4. Right Route 5. Right Dose

13. Performs and documents results of blood glucose monitoring and appropriate intervention.

14. Ensures sterile technique(s) are maintained: Patient and room are ready when the provider arrives. Consent is signed and available. Room and supplies are processed according to EMMC Infection Control policies.

15. Ensures specimens are labeled and processed correctly per policy. .

16. Appropriate patient education occurs post procedure.

17. Immunizations are scheduled and administered according to practice guidelines and documented. (if applicable)

18. Medication errors are reported to provider and supervision immediately

19. Adverse reactions are referred to provider ASAP – do ADR.

20. Walk-in patients are treated according to protocol.

21. Blood pressure checks are performed and documented per policy.

22. Patient Confidentiality is protected at all times.

23. Is responsible for logging refrigerator temps, eye wash station, check emergency lights and fire extinguishers.

24. Ensure practice adheres to TJC, OSHA, and EMMC policies and procedures.

FINANCE

1. Finishes work on time 95% of the time, avoiding overtime.

GROWTH

1. Has documented evidence of continuing education appropriate to level of practice and certification.

2. Present quality data on a monthly basis as well as the quarterly report or as called for by the Office Practice Leader.

3. Attain goals in performance improvement in all areas studied and measured.
