Patient Coordinator
The Patient Coordinator will be every patient’s first point of contact to getting care. The Patient Coordinator will answer phone calls/emails from patients, help patients schedule appointments, act as a liaison between treating doctor and patient to evaluate patient’s progress to manage appointments and treatment plans, and follow-up with patients about missed appointments and documents. The Patient Coordinator is solely responsible for ensuring patients receive comprehensive and compassionate care by consulting with patients to discuss health problems, developing treatment plans to address patient’s healthcare needs, and educating patients about their conditions and medication.
Consult and collaborate with other health care providers and specialists to arrange patient appointments and treatment plans.
Check-in on patients regularly and evaluate and document their progress.
Assist the clinical team with developing and accessing health interventions.
Treat patients with empathy and respect while conducting themself in a professional manner.
Other duties as assigned
Must be able to pass a background check.
High School diploma or equivalent work-related experience
Bilingual – Spanish speaking
Superb verbal and written communication skills in English and Spanish to share updates with patients and other medical staff – must read and write in Spanish and English.
Outstanding customer service, communication, and interpersonal skills to provide ongoing support for patients and their families and work in conjunction with medical center.
Excellent analytical, organization, time management, follow-up, and problem-solving skills.
Must be proficient in Microsoft Office with excellent typing skills.
Must be self-motivated, self-starter.
Must have a general knowledge of basic medical terms.
Must be able to work in a fast-paced work environment with constant multi-tasking throughout the day.
Ability to work an 8 hour shift between the operational hours of 8am – 7pm
