CARE COORDINATOR

The Care Coordinator role functions as a member of a care coordination team providing services for persons experiencing mental health and/or alcohol and other drug Substance Abuse Disorder problems, primarily opioid addiction. The primary responsibilities of the care coordinator include: 1) Support adherence to the patient’s treatment and recovery plan; 2) Manage successful care transitions; 3) Engagement in care; 4) Services may be delivered by phone, face-to-face at the office, in the home, or in the community.
JOB QUALIFICATIONS & EXPERIENCE REQUIRED:
- Two year degree in a relevant social science field or equivalent education and experience. Preferred degree in social work.
- CDCA, Licensed Chemical Dependency Counselor II (LCDC II) or higher or a Bachelor Level LSW or PC.
- Minimum of two (2) years experience in the mental health and/or drug and alcohol field.
- Working knowledge of Montgomery County mental health, drug and alcohol, and other community agencies and resources.
- Working knowledge of The Diagnostic and Statistical Manual of Mental Disorders (DSM- 5), as related to coordination of community support services.
