Care Coordinator – Community Aged Care

Due to the continued growth of our business, Right at Home Townsville are seeking a Care Coordinator who is motivated, reliable, and passionate about aged care.

Right at Home is Australia’s leading in-home care company. We support people living with dementia and cognitive decline, post-operative care needs and older Australians. Our services help clients remain safe and independent in their homes, and our organisation provides these services through a number of funding sources including private pay clients, My Aged Care and NDIS.

About the role:

The successful applicant will be working within a team of Care Coordinators and will be responsible for liaising with clients, family members, staff and a wide range of stakeholders to ensure the services provided to clients are in accordance with the client’s needs, goals and preferences.

The role also includes the assessment of our clients individual needs with subsequent care planning to provide client focused and consumer directed care with high quality outcomes – achieved using a flexible, responsive, creative and collaborative approach.

In addition to care coordination, this role will also assist in the management of inside and outside sales enquiries – by developing and maintaining new and existing referral sources.

Requirements:

• 1 year minimum experience in Care Coordination / Case Management (or equivalent experience in the health / aged care community services sector).
• Demonstrated knowledge and experience in a similar role (Coordinating and delivering consumer directed care services to Govt funded HCP clients).
• Proven knowledge of the Aged Care Act.
• Advanced communication and negotiation skills and the ability to model a positive culture and strong values through a professional service.
• A clear understanding of (and experience in) delivering services according to the Aged Care Quality Standards.
• A clear understanding of the changing and diverse needs of clients.
• Proven relationship management skills with internal and external stakeholders, coupled with good interpersonal skills and style.
• Reliable Vehicle with Comprehensive Vehicle Insurance
• Driver’s License
• Current Australian National Police Check (or eligibility to obtain)
• Current First Aid and CPR (or willingness to obtain)

Qualifications:

• Diploma of Community Services – Case Management (Preferred), or
• Bachelor of Occupational Therapy (Preferred)

Expected Start Date: 22/06/2021

Job Types: Full-time, Permanent

Salary: Up to $75,000.00 per year

Schedule:

• 8 hour shift
• Monday to Friday
