We are seeking additional support to manage our 22 bed residential care facility in Chelsea Vermont. This position is full time with scheduled hours days m-f and on call evening and weekend. Pay Rate: $17 – $28.00 per hour dependent on experience and licensure.
this is two positions, one day one evening.
POSITION SUMMARY
Under the general supervision of the owner, the Administrative Assistant is responsible for a variety of secretarial and clerical and patient oriented functions. This includes resident admissions and discharges, order entry and coordination of activities within the home , with other staff, resident families and physician’s offices. Under the supervision of Charge Nurse or Licensed Nurse provides direct and indirect patient care for residents on the unit.
DUTIES
Prepares the resident chart and/or admission forms. Obtains prior medical records. Enters appropriate demographic data. Obtains signatures and documents as required. Notifies appropriate personnel with needs and changes. Communicates potential problems with appropriate personnel about potential resident needs.
Maintains organized flow of resident admissions, discharges, transfers by: keeping Nurse Manager updated with information, completing resident record, updates and thins charts, closed out resident record at discharge and assists nursing with all required clerical duties.
Insures all physician orders are renewed per schedule and in compliance with State and Federal Nursing Home regulations that pertain to physician orders as assigned by Nurse Manager and Charge Nurse
Answers telephone takes and relays messages. Accurately answers non-medical questions and/or directs the public to the person who can assist them. Calls physicians to make appointments, delivers reports, and notify of new admissions and consults. Assists with placing calls to family members, other medical treatment facilities or providers, clergy, police, etc.
Serves as receptionist to persons arriving at facility. Provides assistance to physicians, other members of the health care team, nursing home, hospital and home health personnel and visitors. Communicates with residents/family members/visitors and staff in a courteous, respectful and professional manner.
Completes all necessary records, logs, graphics, and flow sheets. Prints labs/x-ray results, and MAR’s. Files appropriately. Assures the oncoming shift is aware of any duties that have not been completed. Photocopies charts, insurance cards etc. as requested by physician, nurse or social service staff,
Processes valuables for the resident. Distributes related information to the resident or responsible party. Maintains and updates census information and notifies appropriate personnel about admissions, transfers and discharges.
Arranges off-site resident appointments and arranges or provides transportation to appointments.
Arranges regular visits of specialized health care providers to provide services to residents, for example, vision, podiatry, and dental.
Arranges for repair, replacement and maintenance of resident items such as eyeglasses, dentures and hearing aids.
Obtains special medical services, special equipment and supplies, and personal items for residents when requested.
Operates riverbend car for resident appointments and other facility business.
Protects resident rights to privacy and confidentiality.
Anticipates resident and family needs before they become problematic and utilizes the appropriate resources (owner manager) to address those who appear to be distressed.
Displays a positive attitude toward work and never communicates complaints or negative attitude to the public.
Will assist with the daily care needs of residents, according to their plan of care and within the scope of the individual’s ability. This may include assisting residents with ambulation, feeding, personal care and assisting other staff members to meet the needs of the resident (s). Observes and reports any overt changes in condition or behavior and reports such to the licensed nurse. Documents as appropriate in the electronic medical record.

Prepares patients meals, distributes food , assists or feeds patients as necessary, assist with dishes and clan up. Anticipates patients’ needs whenever possible and responds to patient call bells.

Employees will perform his/her job in a safe manner as defined in any and all applicable policies specific to the job including but not limited to policies addressing: Universal precautions, Use of PPE, Safe lifting/ergonomics, Handling of hazardous materials and all other policies.
Notifies direct supervisor of infractions of policy, procedure, laws and regulations as the are identified. Notifies supervisor’s manager of same if supervisor does not correct or resolve the issue within a reasonable time frame.
Priority to assisting to manage staff, staff scheduling and on call needs of residents and co workers.
All other duties as assigned or requested.
BASIC KNOWLEDGE
Must have strong organization skills and be able to navigate multiple tasks. Is an excellent communicator and works collaboratively with all team members. Possesses an overall knowledge and understanding of the aging process.
EXPERIENCE
Minimum of two years’ experience as an LNA or in a long-term care or residential care or skilled nursing facility.
MINIMUM REQUIREMENTS
Current VT LNA license is desired or lpn. A Motor Vehicle Record (MVR) will be required to be produced by the employee prior to hire. A valid Vermont Driver’s License and background clearance will also be required prior to hire.
