SCN Hiring for Discharge Planner at Shelby Charter Township, MI Full Time

SCN is hiring a Discharge Planner. As the Discharge Planner you will assist and perform social work and discharge planning responsibilities for residents in a nursing facility. The position prepares the resident to transition from the nursing facility to the next appropriate care setting based on their individual needs. The Discharge Planner applies their understanding of medical conditions, coverage, and initiates an individualized discharge plan.

Responsibilities:

· Coordinate meetings among doctors, outpatient care providers, family members and support people

· Completes patient assessments as needed to make sound admission decisions

· Must demonstrate empathy for their patients and work cooperatively to develop a plan the patient is likely to follow

· Able to give a discharge summary to a patient’s insurance provider to secure coverage for his care

Requirements:

· Solid organizational and case management skills

· Ability to set priorities, take initiative and meet deadlines

· Good communication skills, diplomacy and tact

· Good decision-making, conflict-resolution and effective listening skills

· Ability to handle crisis, cope with job stress and maintain confidentiality

· Ability to work a flexible schedule including early mornings, nights and weekends

· Invests a minimum of 75% of work time on referral management, interacting with hospital discharge planning staff daily, providing updates on pending referrals and gathering new referrals

Pay: $60,000.00 – $65,000.00 per year
