Executive, Service Planning & Care Integration (Contract)
You will work in collaboration with patients’ family/caregiver(s), clinic/hospital/specialty care providers, and community resources under the Primary Care Network (PCN) to provide support to the chronically ill patients. These include monitoring the patient’s progress and ensuring timely delivery of holistic care by liaising with various internal and external stakeholders. In addition, you will work closely with the PCN team to manage challenges faced during the care delivery process as well as be a key participant in quality improvement and primary care partnership initiatives.
Requirements:
- Degree with at least 2 years of experience
- An effective communicator across all levels in an organisation
- Good command of written and verbal English
- Able to work independently as well as in a team
- Meticulous and self-motivated team player who is able to multi-task, work under pressure and within tight timelines
- Proficient in Microsoft Office applications
