Community Health Worker (CHW)

Being a CHW means being a bridge between the community and the healthcare system. Community Health Worker understand the complex personal and medical barriers the community faces since they are often either members of the community themselves or have an unusually close understanding of them, making them invaluable assets to the communities we serve and the healthcare system as a whole. They are the eyes and ears of the medical team out in the community. Community Health Worker are individuals who provide guidance, support, and resources to address any aspects of social determinants of health that the community may be experiencing and help patients in a holistic manner.

This position is important to us because the role of a Community Health Worker is essential to the communities we serve. The medical team doesn’t know or understand the things that Community Health Worker do and are often unable to build the same trusting relationships with the community. We strive to find candidates that have a passion in serving the community in order to as that bridge between community members and the health system.

Under the direction of the department supervisor, the CHW may be responsible for providing and coordinating health education and disease management education, assisting individuals with navigating the health and social care systems, and providing case management services (under supervision). The CHW will serve as a bridge between the patient and the medical system by building trusting relationships with community members served by the hospital or program(s). Specifically, the CHW may be responsible for community outreach and providing health education in a variety of settings including health fairs, community events, group sessions or virtually, and other formats as needed. The CHW may also help in identifying barriers to accessing quality care, work with individuals to overcome these barriers, and provide referrals. Home visits may be required for some interventions. SUHI is seeking an enthusiastic, passionate CHW candidate that meets job requirements outlined below. Position Requirements: •Must have high school diploma or GED •Previous experience in health education preferred, but not required •Knowledge of Chicago’s West Side and/or South Side communities (resident of West Side or South Side communities preferred) •Passion for and dedication to working with community members and providers •Punctual, reliable and willing to learn •May require a valid driver’s license and vehicle •Proficiency in Microsoft Word, Excel, Access and PowerPoint preferred If interested, please apply by visiting http://www.sinai.org/careers.
