The Care Coordinators duties will include, but not be limited to:
Provide counsel and advice to immigrant patients regarding eligibility for public benefits.
Screen patients for potential eligibility for public benefits, including CalFresh; WIC, SSI, CalWorks, Unemployment, and other benefits for which they may be eligible.
Advise patients who are concerned about Public Charge issues and referring to legal counsel for additional advice as needed.
Assist patients to enroll in benefits for which they are eligible.
Follow up with patients to ensure successful enrollment.
Refer patients to legal service or other providers as appropriate.
Data input to track work involved in assisting patients to apply for benefits.
Initiate outreach to target population using telephone, text and email.
Establish and develop trust with clients.
Provide administrative support to Director of Legal Services.
