The Grand Rehabilitation and Nursing Center at Guilderland is currently a Charge Nurse.

Periodically review the residents written discharge plan. Participate in the updating of the residents written discharge plan as required.

Charge Nurse Duties Include (but are not limited to):

• Make written and oral reports/recommendations concerning the activities of your shift as required.

• Ensure that the Nursing Service Procedures Manual is current and reflects the day-to-day nursing procedures performed.

• Ensure that all nursing service personnel are in compliance with their respective job description.

• Participate in the development, maintenance and implementation of the facilitys’ quality assurance program for the nursing service department

• Perdiodically review the residents’ written discharge plan

• Participate in the updating of the Residents’ written discharge plan

• Admit, tranfer and discharge residents as necessary.

• Report any known or suspected unauthorized attempts to access facility’s information systems.

  LPN Requirements Include:

• Current and valid NY LPN License

• Experience in a Nursing Home or Hospital

• Good Customer Service Skills
