Purpose: Responsible for group and individual education sessions that focus on strategies for achieving positive health outcomes for individuals living with HIV/AIDS.

Facilitation of Health Education Curriculum
Schedule, develop and plan Health Education curricula and activities for education groups for HIV positive patients and clients.
Facilitate health education groups focused on treatment education, health literacy, and transmission prevention.
Individual Health Education
Collaborate with Care Management, Supportive Services, Clinic, and Behavioral Wellness in assessing patient/client health education needs, and participate in case conferences.
Provide patient/client-centered, quality driven, culturally appropriate Health Education to HIV+ patients/clients in compliance with agency policies and funder requirements.
Conduct a Health Education screening to assess Health Education needs/interests.
Develop a health education plan that includes health education goals, specific activities, and regularly updated needs and outcomes.
Build relationships with patient/client and all members of the treatment team to support continuity of care and promote health.
Coordinate and participate in case review meetings that include all members of the interdisciplinary team as appropriate.
Orient patients/clients to services available within the agency and to the mission at Trillium Health.
Group Health Education
Demonstrate developing knowledge of HIV/AIDS and related subjects
Plan and organize HIV/AIDS and other health education programming for group sessions
Deliver professional quality presentations on sexual health and other HIV/AIDS wellness topics
Compile and report program data/demographics as required by funder(s)
Collaborate with Care Management, Supportive Services, Clinic, and Behavioral Wellness in assessing patient/client needs
Adhere to all government and funder regulations

Qualifications
Associates Degree, B.A. or B.S. preferred, with 2 years of experience working in the field of HIV/AIDS, behavioral health, substance abuse, health education, or other chronic illnesses. One to two years of experience facilitating health education required. Familiarity with regional HIV primary care, mental health, substance abuse and other services and resources. Possess an understanding of community level work and the importance of collaborating and coordinating with other organizations. Effective communication and documentation skills. Cultural and linguistic competence for the target population. Must be accustomed to diversity.
Physical Requirements
While performing the duties of this job the employee is required to stand, sit, walk. Occasionally the employee must stoop, bend and lift or move up to 25 lbs. Must be able to operate a motor vehicle.
