We have a unique opportunity for aVirtual Nurseto join our team as we implement a new patient care model that leverages technology to provide additional support to our staff while also improving patient experience.
As a Virtual Nurse, you will utilize your clinical knowledge, critical thinking skills, and the principles of case management and adult education to coordinate, implement and support a plan of care that meets the patient’s needs and ensures a seamless, effective, and efficient transition of care. This includes the assessment, planning, implementation, coordination, monitoring, and evaluation of patient plan of care from admission to discharge.

GENERAL REQUIREMENTS
Must be licensed in the State of Indiana as a Registered Nurse.
Bachelor of Science in Nursing preferred.
Graduate of an accredited school of nursing.
Certification by a nationally recognized nursing organization is preferred
Basic Life Support for Healthcare providers certification
Minimum ten (10) years of experience in health condition management, which must include adult condition management. Electronic Medical Record (EMR) experience.
