This position is full-time. Employees are required to have flexibility to work any of our 8-hour shift schedules during our normal business hours of 7am – 7pm CST. It may be necessary, given the business need, to work occasional overtime.
We offer 6 weeks of paid training. The hours during training will be 8am to 5pm CST, Monday – Friday. Training will be conducted virtually from your home.

Primary Responsibilities:
Assess, plan and implement care strategies that are individualized by patient and directed toward the most appropriate, lease restrictive level of care
Identify and initiate referrals for social service programs; including financial, psychosocial, community and state supportive services
Manage the care plan throughout the continuum of care as a single point of contact
Communicate with all stakeholders the required health-related information to ensure quality coordinated care and services are provided expeditiously to all members
Advocate for patients and families as needed to ensure the patient’s needs and choices are fully represented and supported by the health care team
Generally work is self-directed and not prescribed.

Required Qualifications:
Bachelor’s Degree in Social Work (BSW)
Licensed BSW with applicable state licensure (where work is performed)
2+ years of experience in care coordination, case management, clinical or community health setting
2+ years experience in a hospital, long-term care, home health, hospice, public health or assisted living
Experience navigating local community healthcare and social services resources
Intermediate proficiency using Microsoft Word (create, edit documents), Excel (create/edit spreadsheet, filter, sort, basic formula), Outlook (calendar management, emails)
Ability to work any of our 8-hour shift schedules during our normal business hours of 7am – 7pm CST. It may be necessary, given the business need, to work occasional overtime.
Preferred Qualifications:
Master’s Degree in Social Work (MSW) Licensed (applicable state licensure)
Experience with electronic charting
Bilingual fluency in English and Spanish
Experience with arranging community resources
Experience managing populations with complex medical or behavioral needs
Telecommuting Requirements:
Ability to keep all company sensitive documents secure (if applicable)
Required to have a dedicated work area established that is separated from other living areas and provides information privacy.
Must live in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service.

California, Colorado, Connecticut, Hawaii, Nevada, New Jersey, New York, Washington or Rhode Island Residents Only: The annual range for California / Colorado / Connecticut / Hawaii / Nevada / New York / New Jersey / Washington / Rhode Island residents is $58,300 – $114,300 per year.
