This position is full-time. Employees are required to have flexibility to work any of our 8-hour shift schedules during our normal business hours of 7am – 7pm CST. It may be necessary, given the business need, to work occasional overtime.

We offer 6 weeks of paid training. The hours during training will be 8am to 5pm CST, Monday – Friday. Training will be conducted virtually from your home.

Primary Responsibilities:

• Assess, plan and implement care strategies that are individualized by patient and directed toward the most appropriate, lease restrictive level of care

• Identify and initiate referrals for social service programs; including financial, psychosocial, community and state supportive services

• Manage the care plan throughout the continuum of care as a single point of contact

• Communicate with all stakeholders the required health-related information to ensure quality coordinated care and services are provided expeditiously to all members

• Advocate for patients and families as needed to ensure the patient’s needs and choices are fully represented and supported by the health care team

• Generally work is self-directed and not prescribed.

Required Qualifications:

• Bachelor’s Degree in Social Work (BSW)

• Licensed BSW with applicable state licensure (where work is performed)

• 2+ years of experience in care coordination, case management, clinical or community health setting

• 2+ years experience in a hospital, long-term care, home health, hospice, public health or assisted living

• Experience navigating local community healthcare and social services resources

• Intermediate proficiency using Microsoft Word (create, edit documents), Excel (create/edit spreadsheet, filter, sort, basic formula), Outlook (calendar management, emails)

• Ability to work any of our 8-hour shift schedules during our normal business hours of 7am – 7pm CST. It may be necessary, given the business need, to work occasional overtime.

Preferred Qualifications:

• Master’s Degree in Social Work (MSW) Licensed (applicable state licensure)

• Experience with electronic charting

• Bilingual fluency in English and Spanish

• Experience with arranging community resources

• Experience managing populations with complex medical or behavioral needs

Telecommuting Requirements:

• Ability to keep all company sensitive documents secure (if applicable)

• Required to have a dedicated work area established that is separated from other living areas and provides information privacy.

• Must live in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service.

California, Colorado, Connecticut, Hawaii, Nevada, New Jersey, New York, Washington or Rhode Island Residents Only: The annual range for California / Colorado / Connecticut / Hawaii / Nevada / New York / New Jersey / Washington / Rhode Island residents is $58,300 – $114,300 per year.
