Position Summary

The Practice Transformation Coordinator (PTC) provides administrative and coordination support to our Practice Transformation teams. The PTC serves on a team dedicated to supporting our external healthcare center agency partners and reports to the Director of Practice Transformation or other supervisors, as assigned. The PTC is responsible for overall coordination and organization for the team. This includes meeting scheduling, minute taking for critical agency facing meetings, logging of data into Salesforce, and managing the distribution of materials to our agency partners.

Essential Duties and Responsibilities

Serve as the coordinator of practice transformation activities and key integrator of information and process intended to drive the delivery of high quality support to healthcare partners. Key responsibilities include:

• Convert received content on agency activities into standardized status reports
• Enter Agency implementation notes, communications, and resources into various data and technical platforms (e.g. Salesforce)
• Drive scheduling/coordinating larger meetings for agency-facing work Managing logistical support and communication for Upstream Training
• Manage distribution of materials to agencies, incl. Patient Education Materials, Sustainability Binders, etc
• Purchase materials, appreciation gifts and supplies as needed
• Enter agency related expenses and grants into budgets
• Support data driven quality improvement through developing expertise in available data platforms, including Azara, IMAT and Salesforce, and retrieving and validating reports for the PI team, leadership, and agency partners
• Develop knowledge and gain agency facing experiences in supporting key meetings such as the program kick-off meeting

Required Education, Experience, Knowledge, Skills and Ability:

• 1-4 years of experience coordinating events, meetings, and/or program activities/projects. Healthcare experience is a plus
• Demonstrated ability to support across multiple projects and supervisors with competing priorities
• Proficiency in G Suite, Microsoft Word, Excel, Power Point, and quick learner of new software/programs
• Excellent verbal and written communication skills with ability to work independently and collaborate with timely coordination with other team members
• Demonstrates effective multitasking, and organizational skills with keen attention to detail to complete competing priorities in timely manner
• Service-orientation/mindset
• Ability to work independently and manage/prioritize workload and be able to foster and maintain relationships with team members, leadership and external agency stakeholders; Possesses highly collaborative approach
• Able to attend work related in-person meetings and functions as needed (Upstream employees are required to be fully vaccinated to attend in-person work-related activities.
