Summary The James A. Haley VA Hospital is a clinical referral, complex level 1A healthcare system. It is a teaching hospital, providing a full range of patient care services, with state-of-the-art technology as well as education and limited research. Comprehensive healthcare is provided through primary care and long-term care in the areas of dentistry, extended care, medicine, neurology, oncology, pharmacy, physical medicine, polytrauma, psychiatry, rehabilitation, spinal cord injury and surgery. Responsibilities Responsibilities: This position will program manage and coordinate dialysis vascular and Peritoneal Dialysis (PD) access placement and tracking for all veterans with or approaching ESRD in our catchment area. DVAC will support renal, self-care, home and in-center dialysis clinics with ongoing patient and staff education and follow up. Dialysis Access Vascular Coordinator (DVAC) will attend additional clinics as needed (surgical and vascular) where they can reinforce education and facilitate coordination of care. DVAC will work in collaboration with the renal nurse navigator, nephrology leadership, outlying VA facility coordinators and general and vascular surgical services to provide timely follow up and guidance. DVAC will monitor the current vascular and PD accesses and be proactive in recommending early intervention. DVAC will also initiate and maintain formal reports and/or present, as necessary the Quality Assurance Performance Improvement (QAPI) data on vascular and PD access and other PI data as directed both locally and nationally as required by VA mandate and regional renal network. In coordination with renal nurse navigator, DVAC will coordinate patient and staff educational resources, educational programs, classes, and referrals. DVAC will participate in research and facilitate special projects designed to evaluate and improve patient care and delivery of care. DVAC will provide leadership as evidenced through effective interdisciplinary problem solving and consultation with other services and will provide active leadership on interdisciplinary committees, task forces or other QI or organizational improvement groups that demonstrate improved patient outcomes. The DVAC will provide care to patients requiring hemodialysis or peritoneal dialysis as a contingency based on the staffing needs of the unit. Purpose: The purpose of the work is to provide supportive care and care coordination to the pre End Stage Renal Disease (ESRD) and ESRD patient impacting the patient, dialysis staff, various clinic staff, outlying facilities that provide us with a patient population of both local and fee-service patients. The DVAC will support renal and dialysis clinic patient population as primary coordinator of dialysis vascular and peritoneal (PD) access placement and will provide guidance, ongoing education, tracking and follow up to the veteran population including inpatients, outpatients, fee-based dialysis patients both locally and within our catchment area and pre-ESRD patients in consult with renal and dialysis clinic MD’s and the renal nurse navigator. Graduate of a school of professional nursing approved by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). The approving official may authorize a waiver of the requirement for ACEN or CCNE accreditation of any degree in nursing provided the college or university has regional accreditation from an accrediting body recognized by the Department of Education at the time of the candidate’s graduation and the composite qualifications of the applicant warrant such consideration. In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Preferred experience: Three (3) to five (5) years of recent dialysis clinical experience is strongly preferred. Two (2) years with the renal/dialysis population required. Recent professional (RN) experience in Peritoneal Dialysis is preferred. Bachelor’s degree in nursing (BSN) is strongly preferred. Board certification as a Certified Dialysis Nurse (CDN) or Certified Nephrology Nurse (CNN) from a VA certifying body is preferred. Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Nurse I Level I – An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II – An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor’s degree in a related field with no additional nursing practice/experience; OR a Bachelor’s of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III – An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor’s degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master’s degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II – A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor’s degree in a related field and approximately 2-3 years of nursing practice/experience; OR a Master’s degree in nursing or related field with a BSN and approximately 1-2 years of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III – Master’s degree in nursing or related field with BSN and approximately 2-3 years of nursing practice/experience; OR a Doctoral degree and approximately 2-3 years of nursing practice/experience. Note regarding MSN degrees: If your MSN was obtained via a Bridge Program that qualifies for professional nursing registration, a BSN is not required. Physical Requirements: This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, and pushing. Transferring patients and objects may be required. Mechanical inpatient lifts are provided. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations, operative or invasive procedures, or home settings. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders.
