Job Description

Under the supervision of the Heart and Vascular Leadership team, coordinates patient care in the outpatient setting to encompass a thorough continuum of quality cardiovascular/electrophysiology care for patients with implanted cardiac devices and external cardiac monitoring devices.

• Introduction of the patient to the health system.

• Serves as a liaison and consistent contact between the patient families and healthcare team throughout the process of diagnosis and treatment.

• Compiles patient’s current health record information, with investigation into current relevant test results, medications, lab results, and health history.

• Works to assist patients in coordination of the plan of care, including proper preparation of pts for tests and procedures, as well as follow up care.

• Interrogates, interprets and programs CIEDs providing full disclosure reports to the providers per protocol for approval and entry to the EMR.

• Monitor and evaluate critical cardiac rhythms and EKG rhythm analysis in-person and from remote transmissions and external cardiac monitor alerts.

• Evaluate and triage remote alerts from all device vendor websites, retrieval and triage of the information, prepares full disclosure reports for provider approval and entry to the EMR.

• Triage and assessment of symptomatic patients in the EP clinic with escalation of care to the provider level when necessary.

• Triaging and management of patient phone calls, in basket messages and communication of results.

• Ability to communicate critical findings to supervisor and/or providers directly and in a timely fashion.

• Ensure charges are entered accurately to include correct diagnosis codes.

• Expectation of continuing education regarding electrophysiology and CIEDs.

• Ensures the timely delivery of quality services for patients

• Establishes, monitors, and revises educational materials for patient teaching. Provides individualized educational materials to patients and families for medications, diagnosis, CIEDs and remote monitoring services.

• Maybe responsible for PI and other metrics to determine effectiveness, quality and safety of the patient’s experience, as well as possible participation in quality programs.

• Thorough knowledge of evidence based / best practice.

• Assists providers with in-basket message clarifications.

• Protects patient confidentiality by promoting professional staff communications.

• Schedule CIED in clinic visits and remotes based on appropriate timing to support safe device follow-up care.

  Required Education / Experience

• 3 – 5 years cardiovascular acute care is with Cardiovascular implantable electronic devices (CIEDs RCES and / or CCDS certification preferred

• RN is required; BSN is preferred

• Cardiac rhythm experience mandatory
