Purpose of Your Job Position
The primary purpose of your job position is to assist in directing the day-to-day functions of the facility in accordance with current federal, state and local standards, guidelines, and DHS regulations, and as may be directed by the Executive Director and Wellness Director, to assure that the highest degree of quality care is maintained at all times.
Delegation of Authority
As the Wellness Coordinator, you are delegated the authority, responsibility and accountability necessary for carrying out your assigned duties. In the absence of the Wellness Director, you are charged with carrying out the resident care policies established by this facility.
Duties and Responsibilities
· Assist the Administrator in planning, developing, organizing, implementing and directing the day-to-day functions of the facility, its programs and activities.
· Assist with facility tours and inquiries
· Ensure all monthly vitals are documented by 15th of each month for all residents.
· Enter/record outside agency reports in PCC and follow up with any concerns and complaints.
· PCC charting upon changes or weekly summary updates on each resident.
· Monitoring POC reports daily including E-MAR holes report, sevens report, etc.
· Monitoring and recording shower schedules and resident ADL’s in POC and PCC.
· Complete weekly medication audit report monitoring expiration dates, low quantities, pharmacy re-orders, etc.
· Assist with the monitoring of monthly wellness report by tracking number of incidents, hospitalizations, re-hospitalizations, incident types, etc.
· Medication proficiencies yearly and as needed.
· Medication and PRN review reports completed and submitted to Wellness Director for January, April, July and October.
· Set up and monitor all resident appointments/transportation set up and needed follow up.
· Participate and hold morning stand-up in addition to ED and WD, or in the absence of.
· Assist WD in entering in incident report and follow up needed 3-4 days after incident. All reports to be report to WD within 24 hours.
· Assist in entering in and monitoring new or changed orders in PCC.
· Manage employee scheduling, monitoring overtime, lunches and clock in/clock outs.
· On-call 24/7 in addition to WD and team leads, or in absence of.
· In the absence of the Wellness Director, represent the facility at and participate in meetings.
· Make written and oral reports/recommendations to the ED and WD concerning the operation of the facility.
· Make weekly inspections of the facility to assure that established policies and procedures are being implemented and followed. Report findings to the ED and WD.
· Participate in facility surveys (inspections) made by authorized government agencies.
· Maintain a good public relations program that serves the best interest of the facility and community alike.
· Assist in setting the tone and atmosphere of the facility.
· Act on behalf of the Wellness Director during his/her absence.
· Assist in holding care staff and other departments accountable, assist with training and orientation of all new employees.
