Community Connector
Part-Time to Full-Time (0.80 – 1.0 FTE) 64 – 80 Hours a Pay Period
Day Shift, Hours between 7:00 am – 6:00 pm
The Community Connector employs community-based care coordination strategies outlined in the Pathways Community HUB model to improve health outcomes for targeted populations. Through community-based care coordination this position will provide support to at-risk clients as they navigate the health care and social service systems in the Winona area. This position aims to increase access to support services, improve the quality and cultural competence of their care, and increase the health knowledge and self-management skills of clients served. The Community Connector has a deep knowledge and understanding of the Winona-area community, and holds lived experience that uniquely equips them to bridge barriers related to culture, race, language, socioeconomic status or other factors. This position requires extensive travelling throughout the community working with varied populations, oftentimes in client homes. This position works in partnership with the Live Well Winona (LWW) Winona Community HUB and works closely with Winona Health staff, area social service organizations and other partners. High school graduate or equivalent; some college preferred. Preference for Community Health Worker (CHW) certificate completion.
Employment contingent upon achievement of CHW certificate within two years of hire. The certificate program is at the discretion of Primary Care leadership.
Upon hire and completion of Community Health Worker certification be able to demonstrate an understanding of Community Health Worker (CHW) health care delivery system and the Pathways Community HUB model. Minimum one-year experience volunteering or working in human services, healthcare, community outreach, health education, or related direct service.
Preference for trauma informed care training.
Demonstrated understanding of various healthy coping skills.
Knowledge of local community agencies and resources.
Intermediate computer and phone skills. Ability to use e-mail and navigate the internet, and perform data entry in a timely manner.
Ability to communicate effectively orally and in writing.
Must have a passion for working in the community to address issues of inequity.
Strong social and listening skills are desired, including the ability to build trusting relationships and work effectively with socially and medically complex clients in a variety of non-traditional settings.
Empathy, patience, and persistence are critical skills.
Ability to communicate collaboratively in a team while managing multiple priorities is required.
Must demonstrate skills in creative problem-solving and attention to detail.
Must utilize effective time management skills, and exercise sound judgement while keeping professional boundaries.
Ability and willingness to work hours outside a standard work week when necessary.
The Community Connector must have and maintain valid driver’s license, valid auto insurance, good driving record and access to reliable personal transportation.
Nonessential Skills and Experience:
Previous experience with expectant mothers and/or children is preferred.
Previous experience working with individuals experiencing poverty and/or housing insecurity is preferred.
