ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned. Evaluates patient and develops plan of care based on specific problems identified. Provides customized therapy treatment in accordance with plan of care. Reassesses and updates plan of care and discharges patient, as appropriate. Documents records in accordance with State of Oregon and department standards. Maintains collaborative communication with referring provider, patient/family and other members of the care team. Instructs patient and family in treatment procedures to be continued at home. Confers with Physician and other practitioners to obtain additional patient information, suggest revisions in treatment program, and integrate occupational therapy treatment with other aspects of patient’s health care. May see patients with the Physician for follow up visits. Assists Physician in referring patients who live far away and need occupational therapy treatment closer to home. Communicates directly with the occupational therapist, providing him/her with appropriate protocol information and recommendations for rehabilitation. Requirements Education and Experience: Graduate of an Occupational Therapy program accredited by the Accreditation Council for Occupational Therapy Education. Prefer 2 to 4 years experience. Experience in Hand therapy setting or Certified Hand Therapist (CHT) preferred for treating diagnosis in upper extremity conditions. Must be licensed to practice Occupational Therapy in the State of Oregon. Summary Slocum Orthopedics is recruiting for a part time benefited, 32 hours per week Occupational Therapist. This position has a surgical and non-surgical caseload treating orthopedic injuries in an outpatient setting. As part of a therapy team, Occupational Therapists work in a team environment as well as independently. This position works on-site at Slocum Orthopedics which allows for easy access to MDs as well as MD co-visit opportunities. CHT required.
