Education and Experience:
- Bachelor’s Degree in Behavioral Health, Psychology, Health Behavior Science, or related field.
- Experience working with adults with severe and persistent mental illness and/or substance use disorders.
- CPR and First Aid Certification.
- Valid driver’s license and permission for a review of driving record to meet insurance requirements.
Responsibilities:
- Professionally participate in team conferencing to create and implement care plans for clients.
- Maintain a therapeutic alliance with clients.
- Refer and link clients to all needed services provided by an outside agency.
- Coordinating independent living assistants, medical and vocational services.
- Work with community agencies to develop needed resources including housing, employment options, and income assistance.
- Provide support and consultation with the client’s family.
- Provide in-home services including ADL training, medication assistance and training, social support, and home management assistance; providing supportive counseling.
- Document and keep other staff, clinical coordinators, and Program directors informed of all changes in the consumer’s mental or social status.
- Carries primary or secondary cell phone as assigned and responds to after-hour and emergency calls in accordance with program directives.
- Provides education and works as an advocate for persons served in the community.
