GENERAL SUMMARY:
Under general direction and supervision from Director, Clinical
Analytics & Decision Support, implements population health management
and quality management solutions, including, but not limited to,
clinical quality and managed care financial reporting analytical
solutions, care coordination operational systems, physician tracking
systems, reporting systems, patient registry systems and associate
interfaces.
ESSENTIAL DUTIES:
1. Works with clinical and operational leaders and clients to identify,
analyze, research, select, modify, or update population health
solutions.
2. Collaborates with Information Technology Dept. on the integration of
current systems and the construction of new information systems to
collect and process data necessary to operationalize population health
initiatives and analytics.
3. Acts as a liason with application vendors in order to locally support
assigned systems / solutions and to achieve identified solution
outcomes.
4. Fully understands the overall systems architecture of the department
and considers this when analyzing and preparing information.
5. Assists with or facilitates meetings, conference calls, web meetings,
presentations, and other group communication methods to
6. Applies statistical methods (e.g., confidence intervals, control
limits, etc.) as appropriate when reporting information.
7. Utilizes peer comparisons, benchmarks, etc. to gauge improvement
potential advises clients of same.
8. Conducts and/or facilitates “drill down” and other ad hoc analyses
of the data based on hypotheses, deductions and inferences derived from
initial report review.
9. “Mines” internal and external databases, seeking and identifying
causes of variation and refining improvement opportunities.
10. Advises regarding the design of meaningful, insightful, compelling
formats for the effective communication of information, customized to
target audience(s).
11. Using various databases, writes reports for use by other ment staff
and leaders for their use in generating data necessary to support
population health management
12. Coordinates the timely production and submission of reports per the
specifications and timetables established by external bodies and
internal leadership. Ensures that pertinent reports are distributed or
are available to persons, committees, etc. that would likely benefit
from the information.
