Community Health Navigator
The Community Health Navigator, works under the direction of the Smart Start Suffolk Director, and is responsible for engaging primarily with pregnant women in their third trimester and families with children up to three years of age through regular home visits. The Community Health Navigator introduces skill building activities related to socio-emotional development, conducting regular assessments and assisting with understanding barriers to enhance overall wellness of the familial ecosystem. The work of the Community Health Navigator Worker is to improve maternal and infant health outcomes as well as help to improve the child’s chances of being a successful student upon entering kindergarten.
Responsibilities:
Guide families through family & child skill building activities related to socio-emotional development in the home or approved public setting
Identify and assist individuals and families with accessing preventive and primary health care services as well as essential supports (e.g., pediatric care, Early Intervention, WIC etc..)
Conduct basic health assessments, assist families with identifying needs, provide basic health information, and make appropriate referrals through monthly home visits
Build, encourage and support partnerships and collaborations among all key stakeholders within the target area (e.g., medical providers, community organizations, schools, local businesses, and government agencies)
Performs other duties based on workload.
Requirements:
Must have a High School diploma, GED or relevant experience
Knowledge of the community and community resources.
Respect for and sensitivity to the needs and rights of others, including those with differing cultural, racial, sexual, or ethnic identities.
Good computer skills, knowledge of Microsoft Office Suite (e.g., Word) & Google products
Bilingual English-Spanish
A valid driver’s NYS license and reliable personal transportation is necessary to fulfill job requirements.
Must have schedule flexibility to include occasional early morning, evening, and weekend meetings/activities.
